This declaration form should be signed by all new members of select vestries within Northern Ireland to confirm that they are willing to act as a charitable trustee of the (already registered) parish and are not disqualified in any way from doing so.
Note, this form is for the Charity Commission NI. New members joining already registered parishes in the Republic of Ireland should use the Charities Regulatory Authority form found here: Trustee Declaration – Change of Particulars (RoI)
