A validation form must be completed for each application and sent to to the Board of Education in Church House Belfast, along with the completed disclosure application form.

Disclosure Application forms are available to download as a PDF file from the AccessNI website (www.accessni.gov.uk).  To download these documents, please click on the Enhanced\Standard Applications link.  There are also guidance forms available on the AccessNI website.
